Most Long Term Care Insurance policies provide coverage for private duty home
health "Personal Care Services" and some provide coverage for "Homemaking
Services".
Navigating the intricacies of your insurance policy coverage can be confusing,

your insurance coverage and coordinating your claim. Once you sign an
Assignment of Benefits authorizing
TAKE CARE to contact your insurance
company on your behalf, our insurance team will verify your benefit coverage
and submit the claim forms for you. If requested, our Client Insurance
Coordinator will make a visit to your home to explain your coverage at
no cost to you.
Some policies are reimbursement polices, while others pay TAKE
CARE directly
for the services provided. There are policies that require an elimination
period. Still others are Medicare supplement policies.
TAKE CARE 's insurance team has developed long-standing relationships with many
local physicians, insurance companies, and their case managers to help
your claim process go smoothly. We take great pride in helping our clients
understand their policy benefits. We work with our clients to customize
their care to maximize the policy benefits and limit the out of pocket
expenses.
All services not part of the insurance policy coverage are the obligation
of the client.
Reimbursement Insurance Policies
A reimbursement policy means that the policy purchased will only reimburse after
the services have been provided according to your policy's provisions. In addition,
some reimbursement policies will reimburse the policyholder. This means that
you, as the client, receive reimbursement for services provided by TAKE
CARE. In turn, you are responsible for paying TAKE
CARE directly.
Elimination Period
The elimination period is a period of time specified by the policy in
which the client incurs out of pocket expense. Once that length of time
has been satisfied, the client's policy will begin to make payment for
services rendered. For a client this means that if your policy has a
30 day elimination period, you must pay the first 30 days of care as
an out of pocket costs and your insurance company will begin paying for
day 31 and beyond
Medicare Supplement Policies
Medicare supplement policies are insurance policies that are secondary to Medicare. TAKE
CARE is a private duty home health care agency that does not provide Medicare services; therefore we are unable to invoice Medicare. We can help clients with the claim process for reimbursement from a Medicare Supplement Policy, although clients under these policies must pay TAKE
CARE directly.
Assignment of Benefits
An Assignment of Benefits is a form signed by the client that gives the insurance company authorization to discuss the provisions of your policy, and also allows them to forward payment directly to
TAKE
CARE for services provided. Under HIPAA regulation, TAKE
CARE 's insurance team will be unable to coordinate your insurance claim process without this essential authorization
