TAKE
CARE provides private duty home health care services in Sarasota and Manatee counties to more than 350 clients. We understand that home health care may be new to you and your family. The process of getting started may seem overwhelming. However, we're here to guide you through this.
Susanne Wise, the owner, has more than 25 years experience as a Registered Nurse. She has hired the best RN Case Managers to help you obtain the individualized care that meets your needs, whether you need long-term assistance in your independent residence or simply need assistance recovering from an injury.
When you choose Take Care you can expect to meet with a Registered Nurse who will discuss with you the appropriate level and frequency of care you require. The Registered Nurse will:
Determine with you the hours of care and the specific duties to be accomplished by the caregiver assigned to you
Provide your physician with a written Plan of Care outlining the Take Care services being provided to you for your physician's approval and signature
Maintain oversight of the Take Care caregivers who provide your care
Stay in close contact with you, your family or other responsible party, as well as your physician in order to insure that we continue to provide you with the appropriate level and hours of care
A Take Care Registered Nurse is on call 24 hours a day, 7 days a week to assist you.

LEVELS OF CARE PROVIDED
TAKE CARE provides individualized care to meet your independent lifestyle needs. This care can be scheduled for as little as one hour a day to as much as 24 hours a day, 7 days a week.

COMPANIONS: Provide socialization and transport of the client to diversional activities as well as do light housekeeping, laundry, meal preparation, and errands. A companion cannot provide hands-on care for the client.
CERTIFIED NURSING ASSISTANTS (CNAs) & HOME HEALTH AIDES (HHAs): Provide hands-on care for the client that includes assistance with bathing, toileting, ambulating, and transfers. They also provide socialization and transport of the client to diversional activities as well as assist with light housekeeping, laundry, meal preparation, and errands.
SKILLED NURSING: Registered Nurses (RNs) & Licensed Practical Nurses (LPNs): Provide physical assessment of a client as well as either medication administration or medication box set ups. They communicate with the client's physician if there is a need for medication review or change. Nurses assist with hands-on care of the client. They also provide socialization and transportation of the client to diversional activities as well assist with light housekeeping, laundry, meal preparation, and errands.
PHYSICAL THERAPISTS: Provide evaluation and appropriate exercises for the client to gain and maintain strength and balance.
