As a registered nurse Geriatric Care Manager for Take Care Advisor, Dana’s knowledge of skilled nursing care and extensive experience in care coordination and care management augment her ability to oversee and manage one’s total health care. Part of the integral process in providing care management is the ability to support and guide clients in navigating the many options within their network of health care providers. Through education and awareness Take Care Advisor’s geriatric care managers inform clients of ways to improve and maintain their current living situation.

Dana, a nurse for more than eighteen years, has collaborated with health care providers to achieve ultimate wellness and safety for clients, from conducting assessments to planning, communicating, managing, and education. Dana has shown her commitment to the field by receiving her certification in case management. The CCM® is the first nationally accredited case manager credential. These experiences paved the way for her strong desire to assist the geriatric population in maintaining a higher quality of life and independence for as long as possible.
