As a nurse geriatric care manager for Take Care Advisor, DaVonne brings her knowledge of nursing care management from the private duty home health care perspective and also lends the client support with her nursing and fieldwork background. DaVonne and Take Care Advisor’s staff assist clients and family members in navigating the many options within your own network of health care providers as today’s health care system and health related issues for seniors are complex and confusing.

As a geriatric care manager, this is where DaVonne steps in to provide various means of assistance ranging from, but not limited to:

• assisting with researching alternate living arrangements
• supervising and coordinating the relocation of client and belongings
• helping with the emotional and complex issues with acclimation to a new home
• being the single point of contact for clients, whether in time of crisis or for coordinating medical appointments
• encouraging and ensuring compliance with doctor orders
• transport and accompany clients to physician and medical appointments allowing for record of visits and conversations that transpired to bridge changes from appointment to appointment
• assist in determining when additional care is needed from home health to Medicare assistance focusing on the client’s safety
• assess home for risks and safety basics
• provide crisis management, whether an ER visit or immediate medical appointment need, a geriatric care manager is by the client’s side

Through education and awareness Take Care Advisor’s geriatric care managers inform clients of ways to improve and maintain their living situation.

DaVonne joined Take Care Private Duty Home Health Care in Sarasota, Florida more than twelve years ago working as a private duty staff nurse, providing immense support both in the offices and in the field. Prior to joining Take Care, DaVonne worked on a medical/surgical unit and also as a charge nurse on an outpatient dialysis unit.
