Office Staff  |  Registered Nurse Clinical Supervisor (Office base: Sarasota| full time)

Position Description

The Take Care RN Clinical Supervisor coordinates and supervises patient care provided by both skilled and non-skilled personnel (caregivers) and reports to the Clinical Director. The RNCS also performs admissions for clients to private home health, develops and implements nursing care plans for assigned clients, performs patient care supervisory visits, ensures progress reports are made to the physician when the client’s condition changes or there are deviations from the plan of care, coordinates with schedulers to assure appropriate caregiver selection to staff patients’ care needs, supervises skilled and unskilled care provided in the home or facility, provides case management in all cases involving nursing or both nursing and therapy care, maintains the clinical record for each client assigned.

Licensure Requirements & Experience

  • Current registered nurse licensure in the state of Florida.
  • One to two years of nursing experience, at least one of which is in home health care.
  • Current CPR certification.
  • Good written and verbal communication skills.

Position Description
Take Care Advisor’s RN Geriatric Care Managers provide oversight and guide older adults in managing all aspects of one’s health care for today and for long-term care. Licensure Requirements & Experience

  • Active and unencumbered Registered Nursing license through the State of Florida
  • 5 years or more of home health care experience either Medicare or Private Duty, experience in both areas is preferred
  • In-depth knowledge of the local medical community and Florida resources, including knowledge regarding Medicare services and coverage
  • Computer experience is required-Microsoft Word, Excel, and Outlook

Office Staff  |  Scheduler I (Office base: Sarasota | full time)

Position Description
The Scheduler I is responsible for maintaining customer service relations with caregivers and appropriately organizing and tracking and coordinating caregivers’ schedules. Take Care uses proprietary software to track and maintain client schedules. Through this electronic system, Take Care’s Schedulers accurately and efficiently schedule caregivers to clients, matching them for compatibility and skill. This position requires a customer-service oriented attitude with utmost professionalism, organization, and communication and proficient computer skills.

Caregiver | Licensed Practical Nurse (LPN)

Position Description
The LPN provides direct nursing care to patients assigned by and under the direction of an RN Clinical Supervisor or the Clinical Director.

Required Qualifications

  • Current practical nurse licensure in the state of Florida.
  • Home health care experience preferred.
  • Current CPR certification.
  • Good written and verbal communication skills.

Caregiver | Certified Nursing Assistant (CNA)

Position Description
The CNA provides personal care services as assigned by and under the supervision of RN/Clinical Supervisor or Clinical Director

Required Qualifications

  • Current certification as a Nursing Assistant in the state of Florida.
  • Current CPR certification.
  • Two-hour initial and one-hour biennial in service for HIV/Aids.

Caregiver | Home Health Aide (HHA)

Position Description
The HHA provides personal care services as assigned by and under the supervision of RN/Clinical Supervisor or Clinical Director

Required Qualifications

  • Successful completion of home health aide competency test (AHCA Form #3310-1007)

OR

  • Successful completion of 40 hours of home health aide training
  • Current CPR certification
  • Two-hour initial and one-hour biyearly (every two years) in-service for HIV/Aids.
