An Assignment of Benefits is a form signed by the client that gives the insurance company authorization to discuss the provisions of your policy and also allows them to forward payment directly to Take Care for services provided. Under HIPAA regulation, Take Care’s insurance team will be unable to coordinate your insurance claim process without this essential authorization.
