Take Care Private Duty Home Health

Take Care provides individualized private duty home health care to meet your independent lifestyle needs. This care can be scheduled for as little as one hour a day to as much as 24 hours a day, 7 days a week.

When you choose Take Care you can expect to meet with a Registered Nurse Clinical Supervisor, as a courtesy, who will discuss with you the appropriate level and frequency of care you require. A Take Care Registered Nurse will:

  • Assist you in determining the hours of care you need and detail the specific duties to be accomplished by the caregiver assigned to you
  • Provide your physician with a written Plan of Care, for your physician’s approval and signature, outlining the Take Care services being provided to you
  • Maintain oversight of the Take Care caregivers who provide your care
  • Stay in close contact with you, your family or other responsible party, and your physician in order to ensure that we continue to provide you with the appropriate level and hours of care

Registered Nurses (RNs) and Licensed Practical Nurses (LPNs)


Skilled Nursing is comprised of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who provide physical and cognitive assessments of the client and report their observations to the client, client’s family, and to the client’s physician, if warranted. A Skilled Nurse may accompany the client to a physician appointment and ensure the physician has up-to-date medical information about the client. The Skilled Nurse educates the client and client’s family about the medications the client is taking.

Skilled Nurses can administer medications as ordered by the physician or set up the physician-ordered medications in a medication box so the client can self administer their own medications if the client prefers. Skilled Nurses do hands-on care

of the client and provide socialization for the client by transporting the client to diversional activities. While the Skilled Nurse’s primary focus is the client’s physical well being, the nurse also does light housekeeping, laundry, meal preparation, and errands for the client.

RN Clinical Supervisors: Training & Specialty

Clinical Supervisors: Take Care’s RN Clinical Supervisors oversee and manage the caregivers who provide the Take Care client’s private duty care. Each nurse has completed 3 to 6 months of extended training with Take Care and independently have worked in the nursing field for at least 10 years prior to joining Take Care. Their expertise, experience, andlongevity support our mission in providing the best in care. The RNCSs perform supervisory visits with clients every 60 days and act as the liaison for caregivers and family members.
