CLINICAL SERVICES CONSULTANT

As part of the Take Care office staff nursing team, Barbara works with both prospective and current clients.

In her role, Barbara is keenly aware of the challenges clients face as they are looking to choose home care. Caregivers are at the top of her list when looking for quality home care as she knows that many clients are concerned that they are inviting “strangers” into their home. She points out that finding the right agency starts with knowing how the agency chooses their caregivers. Take Care’s caregivers are employees of Take Care—not independent contractors—and go through rigorous background screenings; but just as importantly, they are hired with the thought in mind that these are individuals we would want caring for us.

Furthermore, to Barbara, the value of knowing that day or night one of our experienced nurses with full knowledge of a clients’ care needs, is just a phone call away. Having a licensed, private duty home health agency provides clients with the close, trusted connection, whether the care is short or long-term. Knowing that a home health agency has a long established record of providing the highest quality care in your community is so much a part of helping a client to feel comfortable in their decision making.

Additionally, through Barbara’s role, she assists clients in learning about the benefits of working with Take Care—for the client and their families as individuals as well relating the reliability and established reputation of the company as whole. She also conducts the initial assessments of clients and develops individualized care plans, tailored to suit the client’s specific needs with the goal of supporting autonomy in the client’s home setting. An office Registered Nurse Clinical Supervisor then builds rapport and establishes clients’ care with an admission. Barbara communicates and works closely with the RNCS to assists in developing and implementing nursing care plans for distinct clients.

Barbara coordinates clients’ physician appointments, making sure to coordinate calendars with clients and Take Care’s schedules. She also supervises the caregivers who provide care to clients in their homes, assisted living, or skilled nursing locations. Take Care caregivers range in skill levels, from a companion, home health aide, and certified nurses aide to licensed practical nurse and registered nurses. The level of care is assigned appropriately per doctor’s instruction or as dictated in the nursing plan of care. Abby provides direction and on-going guidance to all level of caregivers ensuring that at all times they meet Take Care’s highest standards of care.
