Take Care Advisor’s individualized care plans emphasize preventative care and early intervention. The level of involvement is dependent upon your needs. We also assist in various aspects, but not limited to:

  • Advocate: Attending doctor appointments, overseeing physician orders and instructions, and facilitating coordination of information among multiple medical providers
  • On Call: An RN Geriatric Care Manager is on call via phone, 24 hours a day, 7 days a week
  • Supportive Services: Assisting with pairing and monitoring ancillary support services
  • Communication: Option of updating family members, medical providers, and designated others
  • Living Environment: Assessing the home based on your care needs; Relocation; Transitional Support
