When you choose Take Care Home Health, you have nursing oversight and expert management of your care. Take Care customizes each care plan to the client’s specific care needs. A Registered Nurse from our New Services team will visit with you in your own home setting to listen to your requests and wishes.

During the visit, the RN is able to assess and evaluate the client’s needs and make a recommendation on the appropriate level of care: from one-on-one personal care with a home health aide up to skilled care with a Registered Nurse. This will help the RN to establish and individualize the assistance and care plan.

The RN will then present options that will help promote and achieve goals of optimal levels of support and independence. Care may then start that day, the next week, or the following month. We work with you on the timeline and frequency you desire.
