Meet “Mr. Jones” - Just One of Many Smart, Proactive Individuals Who Choose to Establish a Relationship with Take Care Advisor
Allow us to introduce you to Mr. Jones* and how we’ve worked with him as a Take Care client.
Mr. Jones first reached out to Take Care years ago via phone, when he was searching for an organization where he could form a connection in the event something changed with his health. Should he ever need care, he felt it was important to have an established relationship with a group that could help him.
We would be remiss if we didn’t explain “who” Mr. Jones is. He is brilliant and decisive, organized at home, and involved in the local community. He lives independently at a beautiful home in Sarasota. Overall, he was extremely healthy at the time he first contacted Take Care. He didn’t need us at the moment, but wanted to be proactive and sought to find the right health care partner for the future, making plans when he was perfectly capable of managing “everything” on his own. Mr. Jones simply wanted to plan ahead – to have a safety net in place.
Mr. Jones has a power of attorney (POA) designated, an attorney, and an accountant. Between the three professional advisors and himself, important decisions are made together so this group vetted Take Care Advisor prior to Mr. Jones becoming a client. He has no family, so he felt it was important to create a relationship with an agency that could serve as his voice and advocate should he ever experience a medical emergency. He also understands the benefits of having a medical advocate present to support and guide you during even routine medical appointments. Mr. Jones decided to join our emergency program – part of our service which can be activated once we have an established connection. Having an advocate available to provide a complete medical history, log of current medications, and health care directives was important to Mr. Jones in case of a sudden change in his health or during a potential crisis. This was Mr. Jones’ mindset when he first became engaged with Take Care many years ago.
Unfortunately, now, he has experienced a sudden change in health with the diagnosis of a terminal illness. Because he had an established relationship with Take Care, an RN Care Manager attended those initial appointments with the oncologist, guided him through the biopsy, helped him to process the diagnosis, assisted in utilizing his long-term care insurance policy, and truly held his hand as they updated his end-of-life care plans together.
Mr. Jones is still living with this difficult diagnosis, but he has found comfort in having Take Care by his side every step of the way. Our RN Care Managers continue to attend doctor appointments while Take Care in-home nurses and caregivers help regularly at his personal residence. Recently, hospice also became involved as Take Care Advisor continues to follow and assist with his care management. We are here for Mr. Jones and committed to caring for him as his health progresses.
Through Take Care Advisor’s Geriatric Care Management services, we are available to advocate for clients like Mr. Jones in all settings. Should a client wish to share information about their health with others, we are often the eyes and ears on the ground for a client’s power of attorney (POA), health care surrogate (HCS) or family members. Take Care advocates for clients while they are living at home, assisted living facilities (ALF), or skilled nursing facilities (SNF), and throughout their end of life. Our focus is helping clients maintain their health and well-being, remaining available to navigate any changes related to aging or a health crisis.
Navigating the complexities of aging can take a physical and emotional toll, but for Mr. Jones and so many others, we strive to provide medical expertise, comfort, greater peace of mind, companionship, compassion, and kindness throughout each of our client’s unique journeys.
