What is Take Care Advisor? A Dedicated Team Providing Geriatric Care Management Services
In 2008, Take Care Advisor was created as an extension of nursing services offered through Take Care Private Duty Home Health Care.
We hope that our geriatric care management and medical advocacy services offered through Take Care Advisor provide clients and their families peace of mind and greater comfort when navigating everyday life, all medical conditions and health emergencies.
Because we truly believe “Everyone Deserves the Best in Care,” the Take Care team feels privileged to assist you, and/or your loved ones, with personalized services designed with your quality of life, peace of mind and well-being as our top priorities.
Many clients choose this ongoing advisory relationship to ensure future health care needs and wishes are met by a team they already know and trust. Our medical advocates are available to attend doctors appointments, bridge communication between multiple providers, arrange follow-up labs and manage pre or post-op care, working as part of a comprehensive team led by an RN supervisor.
With phone support available 24/7, this program continues to provide our clients and their families with the comfort that the Take Care Advisor team is always only a phone call away to help guide any change in condition, new diagnosis, or medical emergency.
