Take Care Informs Professional Advisors about Home Health & Care Management Services

Take Care's Founder & CEO Susanne S. Wise, RN, MBA, President Courtney Wise Snyder, MSG, CMC, Vice President Erika Wise Borland, MA, Melanie Jeffrey, R.N., B.S.N. who heads up our New Services team, and Administrative Coordinator Sasha Gipson had the privilege of meeting with many local attorneys, CPAs, financial advisors, and trust officers at a Southwest Florida Estate Planning Council dinner on February 13, 2024.

It was a great opportunity to educate professional advisors about resources available to their clients through Take Care. As we reminded this group:

  • At Take Care, we remain proudly locally, family-owned and operated – with a track record of 28 (almost 29!) years of consistency and the utmost standard of care.

  • We focus on providing in-home nursing services, personalized care and companionship, and medical advocacy for our clients. We have three offices in the region dedicated to serving clients throughout Sarasota, Manatee, and Charlotte Counties, including our barrier islands.

  • While those are the geographic locations we serve, it’s important to understand that “home care” may not be exactly what some envision. Take Care can assist clients in any setting: in a private home, at the hospital, during rehabilitation, or within an ALF/SNF - from a few hours a week up to around-the-clock support, 24/7.

  • We pick clients up after elective surgeries, provide medical assessments, monitor and provide medication management, and much more. We work with all physicians and closely with all the concierge practitioners in the area.

  • One of our Skilled Nurses traveled with a client to Arizona for a grandson’s college graduation. This was a client who needed assistance with medication management that family was not comfortable with. We can assist with the day-to-day necessities of life - even providing support during hurricanes.

  • Take Care Advisor’s team of RN Care Managers support clients navigating the issues and options of aging. These RNs play a patient advocate role. We are here to advocate for clients in all settings, we directly help clients and can also be the eyes and ears for the power of attorney (POA), healthcare surrogate (HCS), and family members. Especially for clients with no living family members, our hope is that each of us as professional advisors provide our clients comfort and peace of mind. Take Care's focus continues to be helping clients maintain their health while also being here to help them navigate changes related to aging, or a health crisis. 

Speaking with this organization, we also wanted to call attention to the important distinction to be made between Medicare providers and what we call “private duty.” Take Care provides “private duty” services – meaning clients pay privately for care. Our services are client driven, not physician ordered. We also specialize in the billing, management of benefits, and utilization of long-term care insurance policies. (We do not accept Medicare or Medicaid). Many clients and their financial advisors value the option of having Take Care help navigate long-term care policy benefits.

The Take Care team appreciated the opportunity to share about personalized home health care and our RN Care Management services with this local organization. We remain grateful to professional advisors who have come to trust us with a referral when their clients express an interest – or have a direct need – for care.
