Private Duty Home Health Care

Care is designed to meet your specific needs, from as little as one hour a day to as much as 24 hours a day, 7 days a week.

Skilled Nursing Caregivers

When your care level goes beyond the basics of our team of personal care caregivers and medical intervention is a crucial component to your daily routine, a skilled nurse caregiver is recommended. Did your physician add an additional medication to your daily routine? Are you tasked with the need to monitor your blood pressure on the hour, every hour? These are just a few examples of when one would enlist the support of a skilled nurse caregivers.

A skilled nurse clinically can anticipate the needs of a client as they’re backed with the nursing education and knowledge of medications and the disease process. Take Care’s New Services team is available 24/7 to answer any questions and assist you with identifying the level of care that is appropriate to your specific situation and need.

Registered Nurses (RN) and Licensed Practical Nurses (LPN)

Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are more broadly defined as skilled nurses. They have the license and education to provide physical and cognitive assessments and report their observations to you, your family, and—if warranted—to your physician. A Skilled Nurse may accompany you to a physician appointment and ensure the physician has up-to-date medical information about you. The Skilled Nurse educates you about the medications you are taking.

Skilled Nurses may administer medications as ordered by the physician or set up the physician-ordered medications. Skilled Nurses are permitted to perform hands-on care and provide socialization by transporting you to diversional activities. While the Skilled Nurse’s primary focus is your physical well being, the nurse may also provide support in other ways such as light housekeeping, laundry, meal preparation, and errands.

Our New Services team works with you to customize your care specific to your needs, be it one hour a day of care or 24 hours a day. Call us today to coordinate a courtesy visit.

Continued Oversight: Training & Specialty

Take Care’s Registered Nurse Clinical Supervisors (RNCS) oversee and manage all of the caregivers who provide our client’s private duty care. Each nurse has completed three to six months of extended training with Take Care and independently have worked in the nursing field for at least 10 years prior to joining Take Care. Their expertise, experience, and longevity support our mission in providing the best in care. A Registered Nurse is on call 24 hours a day, 7 days a week to respond to both client and caregivers.

When you choose Take Care you can expect to meet with a RN Clinical Supervisor, as a courtesy, who will discuss with you the appropriate level and frequency of care you require. They will:

  • Assist you in determining the hours of care you need and detail the specific duties to be accomplished by the caregiver assigned to you
  • Provide your physician with a written Plan of Care, for your physician’s approval and signature, outlining the Take Care services being provided to you
  • Maintain oversight of the Take Care caregivers who provide your care
  • Stay in close contact with you, your family or other responsible party, and your physician in order to ensure that we continue to provide you with the appropriate level and hours of care
  • The RNCSs perform supervisory visits with clients every 60 days and communicates any updates or changes in relation to the client’s private duty home health care to the Take Care caregivers and to the designated family members.
